Update your name on your attorney record

To update your name on your attorney record, please submit the required Name Change Form. This form is also accessible through My State Bar Profile or on the State Bar's Forms page.
State Bar licensees may use this form to request an official name change or update their name with the State Bar of California. This includes completing an official name change, updating a middle name, registering an AKA, and/or removing an AKA.
To complete an official name change, your request must include copies of four identity documents; two identity documents showing your former name and two identity documents showing your new name. You will be required to upload necessary identity documents as part of your submission. (Requirements for middle-name updates and AKA registration/removal are outlined in the form.)
